
The Housing Counselor will be an integral part of our healthcare consulting organization based in Minnesota. In this role, you will be responsible for providing expert guidance as you process applications and the intake process of new clients as well as establish solutions related to housing and accommodation services for clients within the community. Your primary focus will be on ensuring that clients have access to suitable housing options that meet their unique needs while collaborating closely with healthcare providers to enhance the overall patient experience.

Gather eligibility documentation
Submit eligibility requests
Deliver disclosure/compliance documents
Create client driven schedule
Assign Housing Coordinator
Assisting the person to access documentation required for Housing Stabilization Services eligibility
Developing a Housing Focused Person-Centered Plan based on assessment outcomes
Supporting the client in identifying their strengths, needs and wants in housing including cultural requirements and/or preferences
Supporting the client to make an informed choice in their housing transition or sustaining provider
Offering resource information for services that support non-housing related goals as identified in the person-centered planning process
Coordinating with other service providers currently working with the person
Helping the person understand their rights to privacy and appeal information
Annually updating the person-centered plan as it relates to housing
Review Client goals/responsibilities as well as their complaints
Maintain files and program records.
Comply with HIPAA and Data Privacy Practices.
Collaborate with healthcare providers, social workers, and case managers to identify suitable housing options for patients.
Facilitate the referral and placement process for clients, ensuring seamless transitions and timely accommodations.
Address any challenges or obstacles that may arise in the housing process, working to find effective solutions.
Provide education and guidance to healthcare professionals on the importance of housing stability for patient outcomes.
Deliver training sessions and workshops to healthcare teams on identifying housing-related challenges and referring patients to the housing program.
Maintain accurate and up-to-date records of housing assessments, referrals, and outcomes for each patient.
Generate regular reports on housing-related metrics, outcomes, and trends to inform decision-making and program improvements.
Establish and maintain relationships with local housing agencies, nonprofits, and community resources to expand housing options for patients.
Participate in community events and initiatives aimed at addressing housing disparities and promoting access to quality housing for vulnerable populations.
Qualifications
Have a driver's license and transportation to meet with clients when needed
Professional Working experience in speaking on behalf of the organization as well as the client
Familiarity with housing resources, regulations, and programs in Minnesota.
Strong interpersonal and communication skills to collaborate effectively with healthcare professionals, patients, and external partners.
Empathy and sensitivity to the needs of diverse patient populations.
Problem-solving ability and a proactive approach to addressing housing-related challenges.
Organizational skills to manage multiple cases and tasks simultaneously.
Must be able to adapt easily, communicate with the team, follow company standards and protocols, work independently and productively manage their time.
Proficiency in relevant software applications for documentation and reporting.
